Hypertension and Diabetes Control & Prevention Project

177725758-(1).jpgThe Delaware Division of Public Health (DPH) has been awarded a contract from the Centers for Disease Control and Prevention (CDC) to prevent and control diabetes, heart disease, obesity and associated risk factors. Quality Insights is partnering with DPH on this project.
As Quality Insights continues advancing the Delaware DPH's hypertension and diabetes initiative across the state, we are seeking additional practices to engage in this great work. Below are details about the project. Quality Insights encourages you to join the other 100+ Delaware physician offices committed to improving the lives of people with diabetes and hypertension.

Quality Insights has created this web page as a means of disseminating current information to your team members. You will also be receiving regular updates about the project, as well as any related news announcements, resources and educational opportunities related to this project via e-bulletins.  

Benefits of Participation

  • Access to Community Health Workers to pilot specific hypertension and diabetes initiatives
  • Free referral letter campaign referring your patients living with diabetes to the state’s FREE Diabetes Self-Management Program (DSMP)
  • Free referral letter campaign referring your patients living with prediabetes to the YMCA of DE’s Diabetes Prevention Program (DPP)
  • Monthly electronic education modules with downloadable tools for your practice and patients.
  • Monthly Hypertension or Diabetes webinars that are also recorded so you can view as you schedule permits.
  • A chance to be highlighted as a Delaware success story to both the Delaware DPH and the Centers for Disease Control and Prevention(CDC)

Practice Requirements of Participation

  • Designate an individual to act as the primary contact for Quality Insights
  • Treat patients with hypertension or diabetes
  • Submit monthly or quarterly quality reports for NQF 0018 (Hypertension in Control), CMS 347 or 438 (Cardiovascular patients prescribed a statin)
  • Submit baseline data for NQF 0018, NQF 0059 (HBA1C Poor Control), and CMS 347 or 438
  • Participate in a short survey and workflow analysis regarding current hypertension and diabetes processes with the practice.
  • Consider best practice workflow modifications for implementation within your office.
  • Share provided education materials with your staff and, as appropriate, patients.

How Can Your Practice Participate?

Quality Insights requires completion of our Memorandum of Understanding (MOU), which must be signed by practices without an active Regional Extension Center (REC) agreement. Once completed, please email your completed agreement to Ashley Biscardi at abiscardi@qualityinsights.org (preferred) or fax to 877.821.2515. Your participation in this upcoming year’s work creates a foundation for your practice’s quality improvement efforts, as well as prepares your practice for future value–based payment models.

Are You Monitoring and Submitting Your Practice's NQF 0018, NQF 0059, and CMS 347/438 Measures?

One of the top reasons to monitor and submit your National Quality Forum (NQF) data is to assure you are providing optimal care to your patients. Additionally, monitoring your data on a frequent basis can assist your practice in identification of opportunities for improvement.

Other reasons include:
   - External importance for public reporting and payment reform
   - Consistency with/comparable to existing national measure sets
   - Allows for consistent comparison of primary care provider
     organizations or within Accountable Care Organizations (ACOs)
   - A tool to support clinical quality improvement initiatives
   - A tool to support additional financial benefits.
   - Implement processes already established and tested by others
